What is a LIPOSUCTION ?

Liposuction or suction assisted lipoplasty is the removal of fat from under the skin using a small, hollow cannula hooked up to wall suction. It has become by far the most commonly performed procedure and is generally very safe when performed in well trained hands in a monitored environment.

By the time we reach adolescence we are pre-programmed with the number of fat cells we are likely to have. With excess caloric intake and decreased caloric burning our adipose cells swell with excess carbohydrates in the form of stored fat and we get convex distensions or small to medium areas that bulge out with increased fat cell content including the bra line, arms, double chin, male breasts, abdomen, love handles, hips, inner, outer thighs are most commonly performed body contouring procedures.

All liposuction techniques of which there are many, start with a thorough evaluation of skin tone elasticity and expectations. Realistic expectations include a significant noticeable improvement in body contouring and shape but even with modern thermoplasty techniques sometimes the skin tone does not recover and remains lax.

The oldest form of liposuction is suction assisted lipoplasty or syringe assisted liposuction which is using a small, hollow instrument with holes in the end attached to suction. The pressure comes rom a syringe whereas in traditional lipo there is a suction system that removes unwanted fat and convex distensions.

The next technology called power assisted liposuction or PAL like SAL is fast at removing adipose tissue but is not noted for increased contraction or skin enhancement results.

Vaser laser assisted ultrasonic liposuction. This form of energy and thermal ultrasound emits a sound wave which cavitates through sound waves of fat cells. The advantages of vaser ultrasonic liposuction over PAL and SAL are its ability to gently cavitate fat tissue. There is relatively little skin contraction and requires port protectors. Dr. Mulholland was one of the first in Canada to offer Vaser assisted ultrasonic liposuction.

Smart Lipo or laser assisted liposuction is one of the more recent energy assisted lipo techniques. A small laser fiber is inserted into a small and strategically placed incision, pre-liquefies and coagulates the fat and blood vessels decreasing bruising and swelling and pain. In addition the small laser passed under the skin will get some additional skin heating and tightening. Smart Lipo is a very popular technique that Dr. Mulholland performs on a weekly basis. The next technique introduced into the marketplace was radiofrequency assisted liposuction (RFAL). Radiofrequency assisted liposuction is a very effective tool and technique for getting significant three dimensional contractions using the radiofrequency assisted current. The RF current oscillates and moves molecules greater than 1,000,000 cycles per second creating internal friction and heat. This heat gradually destroys pigment cells and tightens skin.

In general, he often will use two or three different technologies on the same person to facilitate the best possible coagulation of fat and tightening of skin.

Recovery from some of the newer energy assisted liposuction includes radiofrequency, ultrasound, laser and thermal. This very short period most patients are in a compression garment and some foam for three weeks where they can increase gradually their activities and at the end of three weeks the garment that they’ve been wearing to compress for 22 hours a day, 7 days a week can be work for half days, leaving it off during the night. After three weeks of half day garment wearing you can go back to full exercise with no restrictions and make sure the area is compressed.

What is the recouperation time associated with LIPOSUCTION?

Bruising and swelling usually subside in 2-6 weeks. The final result takes shape over several months.
