What is a breast augmentation?

Breasts that lack volume and projection can be an inherited condition, or can occur with aging and following breast feeding. As we age there is a natural soft tissue deflation, descent and loss of body and facial fat. Loss of body fat with involution and shrinking of the breast gland can result in inadequate breast volume and projection for many women. Whether your lack of breast volume and proportion because of hereditary causes, normal aging, or post baby deflation, breast augmentation implantation surgery may the appropriate procedure for you.

The incisions during breast augmentation surgery can be made through the armpit using an endoscope and a round or anatomic tear drop saline or small to moderate cohesive gel (“gummy bear”) implants. Other incisions include are made around the areola or under the breast, which is called an inframammary incision. Once the incisions have been selected, an ideal pocket is decided upon. The breast implant pocket may be under the breast gland, under the pectorals chest wall muscle, or a combination of both, called the “dual plane”. The breast pocket is often decided upon according to the characteristics of the patient's chest tissues. Finally, the breast implant itself needs to be selected. There are many types of breast implant shapes and types; round or tear dropped (called anatomic), smooth or textured, saline, low viscosity gel, or highly cohesive, form stable silicone gel implants. It is now possible for a woman to select many different heights, projections and widths within each breast implant classification to achieve a very individualized and natural appearance.

In order to assist in the selection process Dr. Mulholland may use computer simulation surgery to give you the best possible sense of what range of breast implantation surgery results can be achieved. The breast implant surgery itself takes approximately two hours to complete and is most often performed under IV sedation. A general anesthetic is often not required. Most Breast Augmentation patients, who have their surgery performed under a light IV sedation, have no recollection or memory of the procedure. The IV sedation or general anesthesia is always performed by a fully certified anesthesiologist, who is caring for your medical condition while Dr. Mulholland is performing the breast augmentation procedure.

What is involved in a breast augmentation surgery?

Breast augmentation can be performed surgically by: going through the underarm area (armpit), going through the periareolar area (by the nipple) or through the inframmary area (under the breasts).

Breast augmentation can be performed using saline implants (salt-water) or cohesive gel-filled implants (“gummy bear” implants”. These implants come in two shapes, round or teardrop. Dr. Mulholland and his breast augmentation team will help you determine the most appropriate type of implant and surgical approach during your consultation. Medium to larger sized cohesive gel-filled implants can only be inserted inframmary (under the breast). Smaller to medium sized cohesive sized implants can be inserted through the axilla. Saline implants (salt-water) are inserted through a small incision made in the armpit, periareolar or inframmary approached. If your procedure involves going through the armpit, Dr. Mulholland then uses a special telescope, called an endoscope. In both the armpit, periareolar and inframmary procedure, Dr. Mulholland creates an accurate and almost bloodless pocket for the implant, either under the chest muscle, or directly under the breast gland. The teardrop or round shaped, saline (salt-water) implant is then inserted through the armpit, periareolar or inframmary incision and oriented accurately. Generally, a drain is inserted for breast augmentation and the patient may go home approximately four hours after their procedure. The drain is important, as it helps to decrease tissue swelling, turgor and discomfort and is removed by the SpaMedica staff after 2-3 days.

How quickly will I heal and is there pain associated with breast augmentation surgery?

You will leave the clinic and go home in an elastic breast band or soft supportive bra. The breast band or bra help to keep the breasts in the correct position. Initial discomfort is controlled with oral medication. There may be some sutures that need to be removed. Light activities can be resumed almost immediately. Aerobic activities and upper body strength training can be resumed in about 6 weeks. The patient will be shown breast massage techniques, which involve massaging the implants within the breast pockets during the post-operative period, which may reduce the frequency of wrinkling and the risk of capsule formation
