What is a breast reduction?

In general, mammary hyperplasia or large breasts are a hereditary condition that has its onset in adolescence and fully matures in the patient’s early 20s. Large breasts may be aesthetically displeasing to the patient, as well as functionally limiting sometimes accounting for significant neck and shoulder pain, under breast inflammatory changes and economic strain in specialized undergarment support wear. Cosmetic breast concerns are purely for the aesthetic appearance, proportion of the breast which may be excessively large for the patient’s figure, shape and form, but are not generally symptomatic.

There are several different breast reduction techniques, the most common being an approach that requires the incision around the areola and vertical incision extending under the breast. This lollipop incision, or short scar breast reduction, minimizes unattractive scars that can sometimes occur in more extensive breast reduction surgery. Very large breasts do require formal, under the breast scar as well and more tissue must be removed to provide more enhanced proportion for larger breasted patients. Often breast liposuction or radiofrequency assisted liposuction or VASER® ultrasonic lipo contouring of the breast or radiofrequency assisted lipo contouring of the breast (BreastTite®) can be performed individually without any incisional surgery, minimizing the scars for mild to moderate breast enlargement, or can be performed in combination with the excisional treatments mentioned above.

Breast reduction surgery can be performed under IV sedation or local anesthesia and oral sedation and takes less than three hours to perform. Generally, breast reduction patients do not require to stay overnight at our certified, 24 hour overnight plastic surgery facility and they leave 2-3 hours following their surgery. Patients are back to relatively low impact activities of daily living the next day.

The procedure has a high level of patient satisfaction if patients are accepting of the required scars. The lipo contouring procedures such as, BreastTite®, RFAL, VASER® laser and liposuction results of the breasts are quite impressive for mild to moderate hypertrophy if patients do not desire visible or extensive breast scars.

Who is a candidate for breast reduction?

Women who have larger, heavier breasts that are aesthetically displeasing, or do not match their body shape and proportion, or breasts are sagging breasts due to past pregnancies, genetics, or aging and are also too large. If your nipple areolar complexes (pigmented areas around nipples) are enlarged and/or pointing too far downward, combined with breast volume that is too large, you may be a candidate for a breast and nipple - areolar reduction.

What results can I expect from a breast reduction?

You can expect smaller, shapelier breasts. Dr Mulholland’s aesthetic judgment is for a natural appearing, anatomically shaped breast reduction.

What is involved in a breast reduction surgery?

The design of the incisions can vary but usually include incisions that extend around the nipple-areolar complex, vertically down to the fold under the breast and then, in some cases the scar extends horizontally along this crease under the breast.

In many cases, laser, radiofrequency or ultrasonic types of liposuction are used during the breast reduction to remove fat and glandular tissue and help limit the length and extent of the scarring and improve breast and body contours.

The nipple areolar complex is repositioned higher, the excess skin is removed, and the breast is reshaped in a pleasing contour and in a more normal position.

The SHORT SCAR BREAST REDUCTION AND LIFT, is often more cosmetically pleasing and does not include the horizontal scar under the breast. Often this short scar breast reduction procedure can be performed under local anesthesia alone, or together with light IV sedation and is usually combined with liposuction techniques to minimize the length of the scars.

How quickly will I heal and is there pain associated with breast reduction surgery?

You will go home 2-3 hours following your procedures in a support bra with only light dressings over the incision lines. The bra is worn as a "dressing" for 2-3 weeks. Sutures are usually completely removed within 2-3 weeks. Initial discomfort is easily controlled with oral medication. Light activities may be started in 7-10 days.
