Leg Vein Removal

SpaMedica® is the home to Canada's most advanced leg vein management program. Most other clinics that purport to treat leg veins only do so with sclerotherapy or laser. At most leg vein centers, little attention, if any, is paid to the actual diagnosis of the problem that is causing the spider veins or varicose veins of the legs. At SpaMedica®, we offer Canadas only full service, comprehensive leg vein management program. It is a clinical reality that many leg vein patients are actually suffering from undiagnosed reflux (leakiness) from their deeper leg vein system, which regurgitates blood into the smaller, more superficial leg veins causing proliferation of big blue leg veins, varicose veins and smaller spider veins of the legs. If no treatment is delivered to these leaky larger, deep veins, then all superficial treatments, such as laser or sclerotherapy will be doomed to failure, high recurrence rates and high patient dissatisfaction.

At the SpaMedica® Leg Vein Center, we offer comprehensive diagnosis of all leg vein problems before we treat them. We provide all leg vein consult patients with a non-invasive, high tech color doppler ultrasound analysis of their deep leg vein integrity. If we diagnose leakiness of the deep leg veins to the visible superficial varicose veins, big blue leg veins, and superficial spider veins, we can offer all our patients a wide variety in the latest high-tech, non-surgical options for treatment.

Once we have diagnosed the origins of patients aesthetic leg vein problems, we can create a customized treatment plan from our comprehensive Leg Vein technologies which include:

(i) Endovenous Laser Closure of the Deep Saphenous Vein.
This outpatient procedure is designed for leakiness of the deep venous system and replaces surgical leg vein stripping. In this procedure, performed under local freezing, a thin IV catheter is inserted into the saphenous vein and, under ultrasonic guidance and control, a tiny micro-fiberoptic laser is inserted through the IV into the unwanted leaky deep, large leg vein. The laser catheter is inserted into the vein and releases photon energy which painlessly damages and eliminates the leaky saphenous vein, in turn, eliminating the leaking venous blood into the superficial venous system.

(ii) Ambulatory Micro-Phlebectomy
In this outpatient procedure, performed under local freezing, large varicose vein segments are removed using tiny instruments in an almost scarless technique.

(iii) Ultrasonic Guided Sclerotherapy
This outpatient technique uses high-tech ultrasound to locate and more accurately deliver sclerotherapy agents and sclerosing chemicals to the deep leg veins, often resulting in leg vein closure and elimination.

(iv) Multiple Wavelength Laser Leg Vein Removal
SpaMedica® has Canada’s largest selection of aesthetic lasers and wavelengths to remove spider veins and larger leg veins. Effective SpaMedica® laser leg vein devices include a Pulsed Dye Laser, Intense Pulsed Light systems, a 940nm Diode laser system and a 910nm near infrared diode-radio frequency system. The correct laser for the specific leg vein pathology is selected to optimize the likelihood of a successful outcome.

(v) Sclerotherpy
Basic needle based sclerotherapy, performed by one Canada’s
leading phlebology-leg vein experts, Dr. Bill Morton. As an anesthesiologist, Dr. Morton, has been inserting intravenous catheters into veins for over 10 years. Dr. Morton brings to SpaMedica® his years of vein management experience.

Candidates include Women/Men over 18 years who have unwanted leg veins. Large saphenous veins, varicose veins, large or small deep veins (up to 5mm. in diameter), as well as small spider veins of the legs, that are a cosmetic concern, may be treated. Symptomatic leg vein problems that are more than just cosmetic are not treated at SpaMedica®. SpaMedica® leg vein therapies are for primarily aesthetic concerns of the legs. Candidates should not be tanned at the time of treatment.

NOTE: All SpaMedica® Leg Vein patients are treated for cosmetic concerns of their leg veins, spider veins or varicose veins. Although some patients may suffer from pain and/or symptoms from their leg vein disease, SpaMedica® Leg Vein Center is treating the cosmetic concerns with leg vein disease not symptom control (although symptoms often do improve with treatment).

Endovenous Laser Closure can eliminate deep leaky saphenous veins and varicose veins. The lasers and intense pulsed light systems are used to selectively target the blood vessel and induce a reaction that closes off the vessel lumen. Sclerotherapy uses needles, with or without ultrasound localization, to deliver chemical agents that close off leg veins.

All SpaMedica® leg vein patients must undergo a painless comprehensive leg vein ultrasound diagnostic procedure at the time of their consultation to facilitate a detailed and informative discussion of treatment options. The Doppler ultrasound assessment of the entire leg vein system, is painless, takes about 15 minutes and will allow The SpaMedica® Leg Vein Team to formulate a comprehensive treatment program for your leg vein concerns that will have the highest likelihood of successful eradication of the varicosities, large and small leg veins and spider veins of the leg.

At the time of the initial consultation, all patients will have a physical examination and ultrasound analysis and written report of the level leakiness After your diagnostic evaluation, many patients can begin there treatment immediately, both laser or sclerotherapy.

Results are dependent upon many factors: size of the veins; valvular insufficiency (“leakiness” of the deep vein system); colour of the skin;
compliance with treatment plan; and energy level that can be safely used.
Results are dependent upon the vein problem treated and technique utilized. All patients have a customized program designed to optimize the outcome.

(i) Endovenous Venous Laser Closure.
This procedure replaces leg vein stripping as the technique of choice for closure of large and leaky deep saphenous leg veins and varicose veins. This outpatient procedure has a very high success rate of 95% at completely closing off and curing the deep saphenous vein insufficiency.

(ii) Ambulatory Phlebectomy
This procedure removes varicose veins and large blue leg veins. It is highly successful with over 90% successful clearance of varicose veins and reticular veins.

(iii) Laser Leg Vein Removal
The use of lasers, 910nm - Radio Frequency systems, 940nm diode laser, Intense Pulsed Light systems or Pulsed Dye Lasers are used to remove unwanted small leg veins and spider veins. With appropriate management of the deep feeding vein system, the use of these lasers can result in over 75% success in clearing leg veins.

(iv) Sclerotherapy
Sclerotherapy involves the use of injectable chemicals that injure and close off the varicose veins or leg veins. With appropriate diagnosis and treatment of the underlying deep vein disease, sclerotherapy has over 75% success at closing off spider veins, varicose veins and reticular veins.

(v) For the sclerotherapy or laser treatments, most patients require an average of 3-5 treatments, separated by six weeks, to achieve the maximum disappearance of the unwanted leg veins. One to four treatments per area are average. The Endovenous Laser Closure and Ambulatory Phlebectomy treatments are generally very successful after only a single session.

Lasers: The laser or Intense Pulsed Light system is turned on for a fraction of a second. The light emitted by the laser is selectively absorbed by the hemoglobin pigment in your leg veins. The duration of the pulse is just long enough to injure the lining of the blood vessel so that it will close over and fade over the first few weeks after treatment. Skin protection is provided by cooling gel.

Endovenous Laser Closure (ELAS™): The endovenous laser closure or ambulatory phlebectomy are outpatient procedures performed under local freezing anesthesia. Patients can go home that same day and are able to walk and return to normal activities.

The endovenous laser closure involves a tiny, fiberoptic laser being inserted through an IV into the saphenous vein for laser closure. The procedure is performed under local anesthesia and patients can return home that same day.

Ambulatory Phlebectomy: Ambulatory Phlebectomy is an outpatient minor surgical procedure performed for larger visible superficial leg veins and varicose veins. The procedure is performed under local anesthesia, where the larger visible vein or varicose veins are removed through tiny, almost scarless incisions and the patient can return home the same day.

Sclerotherapy: Sclerotherapy is an outpatient needle injection technique using chemical sclerosing agents to eliminate spider and reticular veins of the leg. Treatments take 15-20 minutes and patients can go home immediately.

Laser leg vein treatment is generally a comfortable and well-tolerated office procedure. The laser is more comfortable than sclerotherapy and much faster, so that larger areas of your leg can be treated during each session.

Laser leg vein sessions, or sclerotherapy sessions are booked in 15-minute to 60-minute blocks and you can generally return to work immediately. Several sessions may be required.

Endovenous laser closure or ambulatory phlebectomy do not usually require more than one session to be successful.

Support Hose: After any leg vein therapy, prescription specialized support hose must be worn. The special leg vein support hose will be prescribed for you by your leg vein physician and patients will be directed to authorized stores prior to treatment. Many insurance plans will cover the cost of support hose.

Immediately after sclerotherapy or laser leg vein treatment, the area may be slightly red and puffy. This reaction subsides quickly and some patients may develop a slight bruise that disappears over 8-10 days.

After endvenous laser closure, or ambulatory microphlebectomy, there is significant bruising and swelling. The patient wears a supportive wrap, which is then removed and replaced by the support hose, that are then worn for several weeks.

It is possible to return to normal activities or work immediately.

The treated veins fade over a period of six weeks. If needed, you may have another treatment at 4-6 weeks following treatment.

You should avoid being excessively tanned at the time of treatment and protect the area from the sun for several weeks.

Esthetic leg vein removal and management is considered cosmetic and, therefore, is not covered by insurance. All patients leg vein concerns must be primarily cosmetic.

The patient is responsible for payment.

Consultations: Screening consultations cost $150.00 and include a comprehensive Doppler ultrasound assessment, physical exam, history and creation of an individualized management program. For the outpatient surgical procedures, Endovenous Laser Closure and Ambulatory Phlebectomy, this consultation fee may be applied against the cost of the procedure upon scheduling. For sclerotherapy or laser treatments, $50.00 is applied toward the procedure.
