What is a rhinoplasty?

Nose development is usually a hereditary phenomenon. Nose traits are either developed as you mature, such as an overdeveloped nose length, bridge height, bridge and tip width proportions ,or the nose can be traumatically altered through accident. The common nose shape concerns for patients are over projected bridges, noses that are too long for the facial proportions, tips that are too wide, crooked or irregular noses that are too short, under projected, insufficient bridges or internal nasal septal abnormalities with breathing obstructions. All of these functional and aesthetic external nasal concerns can be addressed non-surgically and surgically through nose reshaping procedures such as rhinoplasty surgery.

At our Cosmetic Plastic Surgery Center, our treatment providers, consultants and Dr. Mulholland will discuss with you all the possible nose reshaping options that are available to you. The consultation and computer imaging will provide you with the information and education you need to make an informed decision about rhinoplasty surgery.

Rhinoplasty surgery can be divided into internal (all incisions and scars inside the nose) or external rhinoplasty (a small incision is placed across the base of the nose called the columella). A rhinoplasty procedure and surgical treatment allows Dr. Mulholland to assess and alter the shape of the nasal bones and the nasal cartilages. A bridge bump may be reduced by filing the bridge down. The width of the nose may be narrowed by bringing the nasal bones closer together. The bridge tip cartilages can be sculpted artistically to provide a narrower, more proportionate tip. The tip of the nose can be better supported if it appears under projected, or reduced if the dorsum of the nose makes it appear over projected. During the surgery, an internal airway breathing procedure called septoplasty is often performed by Dr. Mulholland at no additional cost. This is to ensure that airway breathing, as well as nasal facial aesthetics, are optimized.

A rhinoplasty surgical procedure or nose job surgery takes under three hours in most cases, is performed under general anesthesia at Dr. Mulholland’s certified SpaMedica® Plastic Surgery Center using the highest levels of anesthetic care and safety. Your anesthesia will be monitored by a fully trained and certified Canadian anesthesiologist. In your postoperative care you’ll be monitored by fully trained recovery nurses at Dr. Mulholland’s certified plastic surgery center.

Postoperatively, there is a period of time when there is packing inside the nose to minimize the risk of postoperative bleeding and you’ll be wearing a cast to protect and support the fine nose reshaping results that Dr. Mulholland has achieved.

After a week any small sutures will be removed and most people are able to go back to activities with only minor swelling and bruising. The true shape, appearance and beauty of the nose reveals itself over the next three to six months, and most patients are ecstatic with their results.

The non surgical nose reshaping procedures, called injection rhinoplasty or the five minute nose job, have been pioneered by Dr. Mulholland. Injection rhinoplasty offers patients with the bridge bump, bridge irregularity, crooked nose or insufficiently projected bridge to achieve nasal facial balance proportion through a simple injection of soft tissue fillers such as the sugar gels (Restylane, Juvederm, Perlane, Voluma or particulates such as Radiesse or Scupltra). The five minute nose job injection rhinoplasty is a relatively simple solution for nasal facial imbalance or proportion and does not involve any surgery. The injection takes approximately 30 minutes to perform and there may be some minor bruising or swelling after the injection and patients can usually return to work after a day.

Who is a candidate for rhinoplasty?

If you have an abnormal appearance of the tip of the nose as a result of birth defects, injury or disease or the lower end of the nose, or nasal tip that is cosmetically unattractive or does not “fit” with other facial features, you may be a candidate for rhinoplasty or nose reshaping.

How long will does it take to heal from a rhinoplasty?

The patient goes home with an external cast or splint in place above 2-3 hours after the procedure. The nose may be packed for 24-48 hours. The cast or splint is usually removed in 7-10 days. Initial discomfort is easily controlled with oral medication. Majority of swelling and bruising subsides progressively over 2-4 weeks. External sutures (if any) are removed in 4-6 days. Internal sutures dissolve. Semi-final result is evident in 3 months. Final result is evident in 12-14 months, which is the time necessary for complete tissue softening.

Are there any other procedures that will enhance my rhinoplasty results?

An additional procedure that may enhance the result is a chin enlargement. If you need to improve breathing, septoplasty, or the correction of a deviated septum may also be appropriate.
