How is a tummy tuck performed?

Tummy tucks are generally a combination of removal of loose skin that can be mild to moderate in amount and laxity, as well as excess fat subcutaneous tissue of the anterior abdominal wall.  In general, a specific tummy tuck procedure is selected by Dr. Mulholland and his Toronto tummy tuck patients that fits the anatomical concerns and aesthetic goals of the patient, in addition to assuring optimal removal of extra loose skin without residual laxity or dog ears on the sides.  In general, the skin incision and the ultimate scar that results from the tummy tuck procedure is designed by both the patient and Dr. Mulholland.  On the day of the procedure, Dr. Mulholland has his tummy tuck patients bring in bikini and underwear bottoms they would like to wear that would hide the potential tummy tuck scar.

What are the different kinds of Tummy tucks that could be preformed?

There are a variety of tummy tuck approaches including the mini tummy tuck, the standard tummy tuck and an extended tummy tuck.  In general, the bigger the laxity of skin in the lower and upper abdomen and the more subcutaneous fat, the larger both the incision and the procedure tends to be.

A Mini Tummy Tuck incision can be hidden in almost any mini bikini, thong or underwear.  The mini tummy tuck is designed for small to modest removals of skin and can be combined with advanced modern liposuction techniques, such as SmartLipo and BodyTite to remove upper and mid-abdominal fat collections and fat collections on the sides and flanks. A mini tummy tuck will remove loose skin and will improve the appearance of the belly button without scars or having to create a new one. Figure 2.  Toronto Tummy Tuck patients will often have more than one tummy tuck option with Dr. Mulholland, from less invasive with a smaller scar, to more extended versions of tummy tuck procedures for more significant laxity concerns. Tummy tuck scars and approaches can be very short “mini” incisions, which are about the length of a c-section scar, to very long extended tummy tuck belt lift scars for very extensive laxity and loose skin.

A more standard tummy tuck would have the mini tummy tuck scar and incision extended to the groin creases, but still kept within the hip bone pointers and easily hidden in underwear.  With more standard tummy tuck approaches, there is more loose skin and the elevation of the skin flaps tends to go up to and above the belly button, extending often up to the rib cage.  The standard tummy tuck will usually include the creation of a new belly button and is designed to get the optimal skin and soft tissue contours for patients with significant abdominal skin laxity, usually after bearing multiple children and pregnancies or after weight loss/weight gain. With a standard tummy tuck approach, there is the ability to contour and fashion a new belly button.

Dr. Mulholland has been a pioneer and innovator at achieving very natural belly button results and avoiding the generally undesirable circular belly button scars that most patients don’t like.  Dr. Mulholland deploys a specialized Brazilian-type belly button flap that leaves the belly button scar much less detectable and creates more of a visual “innie” than a circular scarred “innie”, or worse, an “outie” that can often be seen in less skilled before-and-after tummy tuck results.  In general with a standard, extended tummy tuck, the muscle may have been stretched and become lax, accounting for some of the abdominal bulge.  A flattening is achieved by suturing the two central tummy muscles, called the rectus abdominis, together.  This tightening of the tummy muscles provides an internal corset for the support and a flattening of the bulging abdomen that is due to weakening of the muscles, generally from bearing multiple children and stretching the abdominal muscles from weight loss and weight gain.  In the mini tummy tuck approach, generally the belly button is not addressed by direct surgery, there are no scars around the belly button and the belly button is improved in its appearance by a tightening of the skin above and below through a tiny super-pubic incision.

Both the Mini Tummy Tuck and Standard Tummy Tuck approach often will combine advanced modern liposuction techniques, such as SmartLipo(laser assisted liposuction) and BodyTite (radiofrequency-assisted liposuction), which can be performed on the hips, flanks and bra line to give a total, 360-degree truncal contour improvement.

The extended tummy tuck is generally for individuals who have had a tremendous amount of weight loss or weight gain, either from multiple childbearing or just genetic weight gain and weight loss.  In general, the loose lower abdominal skin is very floppy and hangs over the pubic region and the loose floppy skin extends out past the hip bones.  The Extended Tummy tuck incision is still kept just above the pubic area and up the groin crease, but now extends around the flanks to ensure all lax tissue, even on the sides of the flanks, is removed.  Again, in an extended tummy tuck a new belly button is created (an “innie”) and any muscle weakness is sutured together and tightened for an internal corset of support.  With extended tummy tucks, like standard tummy tucks and mini tummy tucks, it is common to combine advanced liposuction techniques such as SmartLipo™ and BodyTite™ to the bra line, hips and flanks to get 360-degree contour improvements.

Who is a candidate for a Tummy Tuck?

Good Toronto tummy tuck candidates are those individuals that have realistic expectations of the kind of skin tightening and tummy flattening outcomes that can be achieved.  Good Toronto tummy tuck candidates are also accepting of the scars, even though they are well placed, that have to be made in order to achieve tummy tuck results.  Good candidates for tummy tuck surgery also need to be medically well with no uncontrolled medical illness and are also accepting of the between one and three weeks of recovery required, depending on the type of tummy tuck selected.

What kind of anaesthetic is used for tummy tuck surgery?

Tummy tucks can be performed under local anaesthesia and oral sedation and can be performed in this fashion in conjunction with liposuction if the procedure selected is a mini tummy tuck and the aesthetic anatomical abnormalities are modest in nature. For the skin only Mini tummy tucks and modern liposuction, no general anesthesia or IV sedation is required, unless requested by the patient. For a Standard Tummy Tuck and Extended tummy tucks, where no mid-line muscle tightening is required, local anesthesia can also be performed. Once midline muscles need to be tightened, patients generally need to be relaxed and it is more common for intravenous sedation or general anaesthesia to be selected.

Is a Tummy Tuck painful?

The question of pain and discomfort following tummy tuck surgery really depends on the type of tummy tuck procedure that is selected.  In Mini tummy tucks, only skin is removed with no muscle tightening. The skin removal is usually combined with or without advanced modern liposuction techniques, such as SmartLipo™ or BodyTite™. Mini tummy tucks generally tend to be relatively comfortable in the post-operative course, because there has been no muscle tightening or muscle suturing performed.  Many Toronto Mini Tummy Tuck and Skin-only limited tummy tuck patients can go back to work after a few days.

More Standard and Extended Tummy Tuck patients, with belly button repositioning procedures do have muscle-tightening procedures performed for an internal corset of support, which is very essential for the aesthetic outcome.  Muscle suturing, manipulation and work comes with more pain and discomfort and, in general, most patients who have tummy tuck procedures with muscle tightening do have a moderate degree of pain and discomfort that is usually easily controlled by anti-inflammatories and narcotics and require two to three weeks off work and heavy activities to recover.

What are the contraindications to tummy tuck surgery?

Contraindications to tummy tuck surgery generally include patients who are unrealistic in the degree of outcome and aesthetic improvement that can be achieved or patients that are medically unwell or unaccepting of the post-operative recovery time that is required.

How long does the Tummy Tuck procedure take?

The operating time varies depending upon the specific type of tummy tuck planned and extent of the laxity and aging. How long is the recovery following a Tummy Tuck?

Mini Tummy Tuck recovery times are generally in the order of five to seven days and high-impact exercise can begin approximately four to six weeks following the procedure.  For Standard and Extended Tummy Tucks, where belly button recontouring and muscle tightening procedures are performed, the recovery is generally in the order of two to three weeks, with high-impact and aerobic exercise activity beginning six week post-operatively.

What will I look like immediately after my tummy tuck procedure?

Immediately after your tummy tuck procedure, your recovery will be in a contour compression garment.  The tummy tuck compression garment is important to re-establish the connection between your repositioned and tightened tummy skin and the underlying tummy muscles, the rectus abdominis, internal oblique muscle and external oblique muscle, as well as your flank muscles.  The compression garment should be worn 22 hours a day for three weeks. Then, it’s usually worn during the day for 12 hours and left off during the night for the next three weeks, for a total of six weeks of compression garmenting.  Initially, for the first few weeks after your tummy tuck surgery, you feel that your tummy and flanks may be more swollen than they were before the operation and this is due to the natural post-operative nature of swelling, edema and delayed lymphatic drainage.  Over a period of three to six weeks, the tummy flattens and the hip-flank contour improves and you’ll notice a very appreciable improvement in your contour.

Over eight to twelve weeks, your tummy and hip-flank contour will continue to improve, clothes will fit much better and lax skin will have been removed and muscle contour enhanced.  It is extremely common for there to be a small amount of fat just above your pubic scar that was transposed from the upper tummy down to just above the super-pubic area of your scar.  This small “muffin top” is unwanted fat from the upper tummy that cannot be removed via liposuction or excision at the time of the primary tummy tuck and, in general, this can be removed with a simple, small, ambulatory liposuction procedure, performed in less than one hour, but at one year post-operatively.After Toronto Tummy Tuck Procedure

How long will the results of my tummy tuck last?
