What is FUE Hair Transplantation to the Moustache, Sideburns and Beard?

FUE or follicular unit extraction is an abbreviation for a procedure pioneered in large part by Dr. Mulholland where automated and robotic rotatory punches are used to precisely extract the follicular units of single, double and triple hair families that share one bulb or root system.  The small 0.8 and 0.9 mm punch holes from the extraction site heal almost imperceptibly, avoiding the very noticeable and aesthetically unattractive linear scars associated with old-fashioned strip graft surgery. For beard hair transplantation, moustache hair transplantation, chin hair transplantation and sideburn hair transplantation, single hair FUE grafts are selected. The FUE grafts are specially selected from different areas of the scalp to match the diameter and characteristics of facial hair and then the FUE single hair grafts are transferred to the missing portions of the moustache, sideburn and beard and delicately inserted through a small specialized needle puncture technique and the hair graft is left to pick up a new blood supply at the bulb. Once the single hair FUE grafts have picked up a blood supply and are viable, they are permanent, ensuring its long-term survival and production of hair creating a moustache, sideburn and beard pattern.

How is FUE Hair Transplantation to the Moustache, Sideburn and Beard Performed?

The procedure starts with a selection of FUE Hair grafts from specific areas of scalp where the diameter and appearance of the hair shafts most closely matches those of the moustache, sideburn and/or beard, ensuring the most natural appearance.  The number of FUE single hair grafts needed for the restoration is estimated before the procedure. It may be between 250 and 3,000 depending upon the size and density of grafts needed to provide the fullest and most natural result. The harvesting sites are shaven to a short stubble (to avoid clogging up the automated systems) and if the hair above the harvest site is long it can cover up the donor site immediately with long hair and/or the addition of hair extensions, or some patients completely shave their head prior to the procedure.  Local anesthesia is inserted delicately into the donor site harvest area, rendering the entire scalp completely anesthetic with no feeling.  While the scalp is anesthetized the small rotatory and automated punches are guided by Dr. Mulholland’s SpaMedica expert hair technicians and the follicular units are harvested.  The small resulting defect from the harvested single hair FUE grafts is less than a millimeter in diameter and heals uneventfully over the next two weeks and is inapparent at normal conversational distances.  This avoids the unattractive and anesthetically displeasing linear scars associated with the old-fashioned strip graft technique.

Once harvested, the follicular unit grafts are kept on ice while the recipient site of the moustache, sideburn and/or beard is prepared.  The area to be grafted has been demarcated and marked out with a surgical marker in conjunction with the patient to ensure the aesthetic goals are achieved.  The area is then again rendered painless with local anesthesia and the transplanted FUE single hair grafts are placed in a precise density and fashion to create a natural looking moustache, sideburn and beard.  A tiny needle is used to create the recipient slits into which the FUE single hair grafts are placed. Micro jeweler forceps are used to painlessly and traumatically insert each little single hair FUE graft into its recipient slit.  Once placed expertly the bulbs of these transplanted grafts pick up a blood supply and begin producing hair.

The donor site, where the hair has been taken grows out over 7-10 days and the stubble hides the now completely healed over small donor site rotatory punch defects.  At that point a barber can blend in the donor site hair length so it all looks uniform.  The area that has been transplanted remains somewhat swollen and there is a dry scab on each shaft.  The dry scab comes off at approximately 7-10 days and about 50% of the hair shafts come off with the scab.  The bulbs, however, do not fall out and retain their blood supply and survive and begin to grow hair at about 4-6 months.  Approximately 50% of the hair bulbs do not shed their shaft and these start growing within 2 weeks and contribute to the density of the moustache, sideburn and/or beard almost immediately.

Who is a Candidate for FUE Hair Transplantation to the Moustache, Sideburn and Beard?

The amount of time taken for the FUE hair transplantation to the beard, moustache and side bun procedure depends on how many grafts need to be harvested and inserted.  Small segmental defects of the moustache, sideburn or beard may take a matter of an hour and a half to two hours for the entire procedure.  However, for full moustache, sideburn and beard restoration, it will require up to 3,000 follicular unit extraction grafts to be harvested and this may take 3-4 hours.  Slow meticulous and artistic implantation technique means another 4.5-5 hours of implantation. Most 3,000 follicular unit extraction cases be it moustache, sideburn, beard or scalp will take between 8 and 10 hours to complete.

Most patients are very comfortable with the oral sedation provided and are able to sit in a semi-reclined position during the implantation and are able to watch movies on a large flat screen or listen comfortably to their own music or the music provided by the clinic.

Most patients find the procedure quite acceptable and not unpleasant.

How long is the recovery following FUE Hair Transplantation to the Moustache, Sideburn and Beard and what will I look like?

The recovery following the FUE hair transplantation to the region of facial hair loss is really related to the conspicuous visibility of the shaven donor site.  Patients who have very long marginal hair above the harvest, will allow that hair to fall over the harvest site completely camouflaging the donor site immediately.  Hair extensions can also be fashioned to cover the donor site or the entire area can be cut very short.  In general, the scabs are off the donor site in 7-10 days and the small 1 mm rotatory punch defects are not visible.

The recipient site looks quite swollen and red for the first 3-5 days and scabs form where the hair shaft is exiting from the donor site recipient slit.  The scabs fall off after 10 days and about 50% of the hair shafts begin to grow immediately contributing to the density of the moustache, sideburn or beard,  while 50% of the hair shafts fall out with the scab.  Because the hair shaft fell out does not mean that the bulb has died.  An overwhelmingly large proportion of the hair bulbs that shed their shaft survive and pick up a blood supply and begin to produce hair shafts by the third or the fourth month.

Early and immediate hair shaft growth then combines with the density of the secondary growth and by one year patients can expect to see their final result.
