What is a the Perineum?

The Perineum is the soft tissue skin and muscle between the posterior wall and entrance to the vaginal canal and the anus and rectum

Why might I benefit from procedure on my Perineum and what is a Perineoplasty?

The vaginal and perineal floor are comprised of a network of skin and muscles, and these muscles can be traumatized with traumatic vaginal childbirth or a surgically-assisted vaginal birth through episiotomy.  Inaccurate restoration of form and muscle function following a traumatic childbirth or a surgically-assisted childbirth can result in laxity of the muscles of the perineal floor.  In its extreme form, abnormal connections resulting from a tear during childbirth can exist between the rectum and the vagina or even the bladder and the vagina, leading to fistulas.  These fistula’s, or abnormal connections and passage ways is a functional problem of extreme significance and the procedures generally is corrected in a hospital-operating environment and coverage by your health plan.  Additionally, prolapse or descent of the uterus out of the vaginal opening can occur with excessive trauma during childbirth. Prolapse fo the uterus is also surgically corrected in a hospital environment and is generally covered by your health insurance plan.  For isolated and significant laxity of the vaginal canal, a Surgical Vaginoplasty is performed.  In very mild Vaginal Laxity Syndrome, non surgical Laser Vaginal tightening and rejuvenation can be preformed with high tech SpaMedica laser and energy devices such as the Mona Lisa Touch, the CORE Intima, the V-Tite, ThermiRF and VA, FORMA V and Fractora V. If this laxity is significant and extends into the perineum, a Perineoplasty can be combined at the same time as a Vaginoplasty. The Perineoplasty involves surgically tightening and repairing the muscles of the floor of your lax perineal muscles.


What is the difference between a Perineoplasty or Vaginoplasty?

A thorough consultation and assessment by SpaMedica Gynaecology Director, Dr. Sasha Syvstonyuk will include a thorough examination of the Mons Pubis, clitoral hood, labia minora and majora, vaginal canal, cervix and perineum.  Excessive laxity of the vaginal canal and the perineum may require a Vaginoplasty or Perineoplasty, and Dr. Sasha will be in the best position to discuss this with you. A Vaginoplasty is tightening the muscles of the floor of the vagina only, where a Perineoplasty involves tightening of the muscles of the floor of the pelvis, including the vagina, but also extending back forwards the rectum.

How is the Perineoplasty performed?

Laser Vaginal Tightening is a non surgical treatment of mild to moderate vaginal laxity syndrome and there is no anaesthetic and minimal recovery.  For more significant laxity, the muscles must be surgically tightened.. Enhanced minimally-invasive techniques such as CO2 surgery diminishing bleeding and bruising and electro cautery radiofrequency devices can minimize downtime following vaginal surgery and are deployed..  However, dissolvable sutures and surgical repair is required which will result in some pain in recovery.

How quickly can I return to normal activities following a Perineoplasty?

Generally, following a Perineoplasty,  a return to light activities such as walking and a sedentary job can occur within several days to a week.  High-impact activities such as jogging, high-impact exercise and sexual intercourse generally require six weeks of recovery time.

How painful is a Perineoplasty?

Generally, a Perineoplasty procedure is performed under local anaesthesia and IV sedation so there is no surgical pain or awareness.  The postsurgical pain is one of bruising and discomfort but less, in general,  than that experienced during  childbirth.

What kind of results can I expect from my Perineoplasty?

Perineoplasty is often combined with Vaginoplasty for the treatment of excessive vaginal relaxation syndrome, and generally most patients are very happy with the pleasure of return of resistance and tautness during sexual intimacy.

Perineoplasty and Vaginoplasty can be combined synchronously with vaginal laser rejuvenation of the vaginal lining using the MonaLisa Touch, the Syneron CORE Intima, the Fractora V and the V-Tite.  This will improve tightness and Atrophic symptoms (dryness, discharge, pain and inflammation)  of the vaginal canal, as well as laxity of the vaginal floor and perineum.

These adjunctive techniques, in addition to rejuvenation of the introitus, the labia minora and labia majora, with or without reduction, fat grafting and fractional resurfacing can be discussed at the time of your consultation.

How much does a Perineoplasty cost?

Perineoplasty is usually performed in combination with Vaginoplasty and generally will cost $4,500 to $5,500 depending on the extent of repair required.  Adjunctive techniques such as Laser vaginal Rejuvenation of the vaginal canal, labiaplasty, fat augmentation, fat grafting and G-spot injection can be combined will be additional costs and will be discussed at the time of your consultation.
