What is the Mons Pubis?

The mons pubis is the collection of fat that sits on top of the pubic bone.  The mons pubic fat pad and the skin overlying it bear the pubic hair and add protection to the pubic bone and region.

What Causes the Mons Pubis Fat Pad to Enlarge or Sag?

With age and multifocal fat collection, fat can accumulate in the mons pubis. The mons pubis can enlarge and form an unsightly bulge in tight clothing, tight undergarments and workout attire.  With age and weight loss the mons pubis can also develop laxity like in other parts of the body, gravity and relaxation of soft tissue ligaments can cause descent of the mons pubis and an unsightly, unattractive sagging. 

The mons pubis can also appear enlarged after cosmetic plastic surgery.  Liposuction of the abdomen or a tummy tuck can result in significant tightening of the abdomen and then once healing has occurred the mons pubis can look relatively enlarged compared to the flatter abdominal contour.  It is sometimes important to plan mons pubis reduction synchronously or at the same time as abdominal contour improvement.

How are both procedures performed?

An enlarged mons pubis or excess fat of the mons area can be treated well with modern liposuction techniques.  Under local anesthesia using BodyTite radio frequency assisted liposuction or SmartLipo or VaserLipo the fat of the mons pubis can be coagulated and liquefied and then gently removed.  This causes a deflation and flattening of the mons pubis. 

The modern liposuction technique such as BodyTite and SmartLipo produce a safe and quantified internal heat, which ensures contraction of the mons pubis.  The contraction and the reduction of the mons pubis the aesthetic goal is to create a uniform contour between the mons and the lower abdomen so that this region looks smooth in clothing and undergarments.

How is a Mons Pubis Lift Performed?

The mons pubis that may have expanded with increased fat and become ptotic or saggy or the mons pubis that is just saggy can undergo a mons pubis lift procedure.  A mons pubis lift procedure is performed under local anesthesia as an outpatient similar to a mons pubis reduction through modern liposuction techniques.

The mons pubis is reduced through liposuction techniques if required and then a lift procedure is performed through an incision just above the pubic hair approximately the size and length of a C-section.  The excess mons is removed and elevated and the mons pubis repositioned in a more youthful position. 

The mons pubis lift can elevate the droopy fat pad of a mons as well as provide increased tautness of the labia majora, minora and the clitoral hood.  Many patients report not only improved contour and shape of the mons pubis, but enhance sexual satisfaction with repositioning and increased tension on the clitoral hood, clitoris and labia majora and minora during intimacy. 

Is it Painful?

The procedure is performed under local anesthesia.  There is a very moderate degree of post-operative swelling and discomfort, but most patients describe the procedure and the recovery is very tolerable. 

There is the expected swelling of the mons in the labial region, which resolves over 2-3 weeks.

What is the Recovery Process?

Walking and light activities can be resumed immediately.  Most patients can return to work in 2-3 days.  There is swelling and bruising and mild discomfort, but patients in sedentary jobs that aren’t very active can return to work in a few days. 

High activity jobs may require a week off.  Patients can return to light activities in the gym such as elliptical and stair climber and isotonic and isometric exercises usually within 3 weeks and high impact aerobic activities and weight training can begin in 4-6 weeks

Is a Control Body Contouring Garment Required Following Mons Pubis Lift and Mons Pubis Reduction?

A lipo-contouring garment similar to that one would wear following Liposuction of  the abdomen is recommended.  The mons is maintained under some compression the vast majority of the day for 3 weeks.  Additional foam padding under the garment is used to compress the soft tissue, which quickly enhances contour and resolves swelling and edema.

After 3 weeks the garment is generally not required and support garments such as an exercise bike pant or Spanx that would be worn for the next 3 weeks.  High impact activities and exercise can resume at 6 weeks.

What Are the Associated Risks?

The risks are minimal and any complications generally minor could be managed with post-operative manipulation.

Are Drains Required Following a Mons Pubis Lift and Reduction?

The mons pubis reduction generally does not require any drainage.  Mons pubis lift depending on the extent and whether it’s combined with other procedures may require a small drain.

What is the Cost?

The cost for reduction is variable depending upon the size of the mons and whether other procedures are performed.  Generally, the reduction itself would start from $4000.  The procedure can be performed under local anesthesia in approximately an hour and half and patients can go home afterwards. 

A mons pubis lift is more involved and involves an incision and the drain is often combined with a lower abdominal tightening; the cost starting from $6000.

What Results Can I Expect?

Patients can expect excellent contour of the mons with flattening and elevation of the soft tissue contours, looking better with and without clothes.
