What is PRP (Platelet Rich Plasma)?

PRP stands for Platelet Rich Plasma. Platelet rich plasma is a sample of patient’s blood that is centrifuged, spun down and prepared to extract special components of the blood, special growth factors and stem cells that can provide rejuvenative and restorative capabilities to various human tissues including hair, skin, anti-aging properties and vaginal and peri-vaginal enhancement and rejuvenation. PRP is also known popularly in the media and by the lay public as the “Vampire lift” (when injected), or the “Vampire facial” when applied topically and combined with other fractional techniques. Toronto PRP, Vampire lift and Vampire facial patients come to SpaMedica for their PRP treatments because of the long and pioneering
reputation that Dr. Mulholland has with the procedure.

What is PRP Vampire Lift

Figure 1. Platelet Rich Plasma (PRP) tube, following the draw, the blood sample is spun in a centrifuge producing the clear yellow liquid at the top which contains the platelet rich plasma (platelets, growth and stimulation factors and stem cells) and then the white separation gel which keeps the red blood cell fragments at the bottom separate from the PRP. The PRP is then drawn off and used for your rejuvenation. Platelet cells are critical in the bodies healing process, whether from a cut or injury, it is the platelet cells that arrive at the site of injury and the various growth factors heal, repair and restore. At SpaMedica, Dr. Mulholland and his team harvest the power of those platelets to rejuvenation you skin, facilitate and enhance the growth of your hair and/or rejuvenation the lining of the vaginal mucosa.

What Can PRP be Used to Treat?

Figure 2. An Eclipse high tech centrifuge is used to spin the blood into its components. The bottom of the test tube contains the dark red blood cells and serum (which is not desirable), while top clear yellow layer contains the platelet rich plasma, which has the growth factors, platelets and stem cells. The clear Platelet Rich Plasma (top layer) is gently drawn off, in a sterile fashion, into a test tube and is ready for use topically or as an injectable PRP.

PRP can be applied topically or injected in various areas of the scalp, face, neck, hands for skin rejuvenation and the body, including vaginal areas to provide rejuvenation, enhanced cell differentiation towards more youthful cells, which will bring out dormant cells such as increasing hair density in male pattern and female pattern hair loss.

Hair densification (increasing hair growth and density) can be achieved with PRP injected to the scalp, with or without micro needling, and low-level light therapy can provide a significant stimulation for dormant cells that have been thinning or falling out following male or female pattern Alopecia. The various cellular fragments and growth factors and stem cells within the platelet rich plasma stimulate dormant follicles to produce hair shafts and producing follicles to produce thicker more aesthetically appealing hair shafts.

PRP is used by Dr. Mulholland and SpaMedica for a variety of rejuvenation and soft tissue enhancement indications including:

(i) Topical administration with or without micro-needling (Vampire Facial), fractional laser resurfacing or radio frequency resurfacing

(ii) Injectable PRP (Vampire Lift) under the skin and within the skin

(iii) Injectable PRP into the scalp for hair growth

(iv) Injectable PRP in to the Vaginal canal for Vaginal rejuvenation

(v) Injectable PRP in the G-spot and O-spot for enhanced sexual satisfaction

How the PRP Treatment is Performed

PRP Treatments, Vampire Lift and Vampire Facials

Toronto PRP and Vampire lift and Vampire facial patients will have their blood drawn, as if you were getting a blood test from an arm vein. Prior to drawing the blood, a topical anesthetic cream may be used on the skin to minimize the discomfort of the small needle and then blood is drawn into a special PRP vile. The PRP vial is then placed in a special centrifuge to be spun down and prepared by experienced SpaMedica technicians. Once the blood vial has been spun, the various components for the blood have been separated out into layers. Various fragments of the blood are then separated and removed, including the platelet rich segment, which contains the stem cells and platelets, which are special blood cells important in the repair of damaged vessels and clotting.

The platelet rich plasma also contains many other growth factors important is new tissue regeneration including:

1. Epidermal growth factor

2. TGF beta

3. Neovascular and angiogenic growth factors

4. Undifferentiated stem cells

The platelet cells, stem cells and the various growth factors once separated and placed in a sterile syringe can applied topically (Vampire facial) or injected (Vampire lift) into the skin, or dermis or under the skin and can provide enhanced tissue, repair and rejuvenative features and tissue.

The PRP Treatment Process

There are three steps to the PRP treatment process. First is the harvest followed by the preparation and then the application.

STEP 1: The PRP Draw

The harvest requires blood to be drawn. Most SpaMedica Toronto PRP patients have had blood drawn before and realize this is a relatively comfortable procedure, performed efficiently and effectively. SpaMedica and Toronto PRP, Vampire facial and Vampire Lift patients come early and get a topical anesthetic cream placed on the inner elbow ante-cubital fossa which is the most common area for blood to be drawn. This topical anesthetic patch can greatly diminish or eliminate the pain for the PRP blood draw. SpaMedica physicians and nurses require a requisite number of tubes for the PRP procedure being performed. A small band aid is placed over the PRP harvest site and this can be removed in a day. There is rarely bruising, swelling or pain after drawing the blood and, so, it is essentially almost painless post-treatment.

STEP 2: The PRP Preparation

The PRP preparation phase is painless and during this phase addition skin, vaginal of scalp treatments may be performed such as mico-needling, fractional CO2 resurfacing and/or Fractional Radio-frequency resurfacing.

STEP 3: The PRP Application

The PRP can be applied topically (Vampire Facial) following a fractional laser or Radiofrequency needle or micro needle fractional treatments of the skin. The application of PRP topically is painless. Three Fractional laser or needle techniques have some discomfort associated with them, but the application of topical anesthetic cream and an air chiller can render this portion of the treatment almost painless, PRP can also be injected in the sub-dermal space with a very tiny blunt micro-cannula and this is generally very comfortable.

PRP injected into the scalp for male or female pattern to treat hair loss can be rendered quite comfortable with a topical air chiller and special SpaMedica technique with micro needles.

Vaginal PRP and peri-clitoral PRP injections for vaginal rejuvenation can be rendered quite tolerable with topical anesthetic cream. Similarly, penile injections are rendered acceptably comfortable with topical anesthetic cream and a chiller.

Topical PRP and Vampire Facials

For Skin Rejuvenation and Anti-Aging

Vampire Facial Skin Rejuvenation and Anti-Aging

Figure 3. Aged skin is characterized by lines, wrinkles, laxity, being crepe, decreased elasticity and brown discoloration. Sun damaged, thin, wrinkled and discolored skin can be greatly enhanced with Fractional Micro injury of the skin (micro needling, fractional lasers and.or fractional RF) and then applying PRP topically so that the platelets, stem cells and factors can move easily through the fractional defects into the deeper layers of the skin, where the RPR can exert its effect.

PRP can be applied topically after fractionating treatments for the skin. SpaMedica Fractional rejuvenation treatments include micro needle systems, micro needle RF systems and Fractional CO2 and Erbium lasers. The mechanical, laser or RF fractionation itself stimulates the skin with new collagen elastin after creating a fractional defect in the skin. With the fractional defect there is a direct communication with cells within the skin and just under the skin following topical application of PRP and is an ideal serum to apply after fractional resurfacing. The Fractional injury creates channels in the skin, scalp or vaginal mucosa and the PRP, which would normally be blocked by intact skin, is allowed to flow through the channels and gain direct access to the deeper cells and levels of the “under skin” . The Fractional injury (laser or micro needling) creates the inflammation in the skin that is then potentiated and harnessed by the PRP to augment and hasten the wound healing response. After fractional resurfacing the PRP can be applied topically for immediate penetration to the skin where the various growth factors and stem cells of the platelet rich plasma stimulate new collagen elastin and ground substances to rejuvenate significantly the skin over and above what fractional treatments can do alone. So the Fractional lasers provide skin tightening and wrinkle reduction alone and this response is amplified and enhanced by topical PRP.

Figure 4. Mechanical Micro needling is performed under local anesthesia. The tiny needles penetrates the skin. The micro mechanical injury created by the needles induces some collagen and ground substance production and new, healthier skin. However, while the channels are still open, topical PRP, growth factors and stem cells are applied and the PRP and its factors and cells greatly amplify the collagen production, skin rejuvenation and skin renewal.

Figure 6. Fractional resurfacing of the lower lid (with laser or RF-radiofrequency) combined with PRP can result in significant lower lid wrinkle improvement and tightening.

Toronto topical PRP skin rejuvenation patients can expect a significant improvement in skin quality. Topical anti-aging PRP (Vampire Facial) when combined with fractional micro needling, fractional radio frequency micro needling, Fractional RF and Fractional CO2 and Erbium laser can have significant results. The fractional mechanical treatments with the needle on laser devices provides rejuvenation themselves, but the additive benefit of PRP and the growth factors and stem cells significantly improves the quality of the elastin collagen and ground substances produced. Generally, a single treatment protocol is deployed and then micro needling is performed once every four months with topical PRP and injectable PRP as an adjunct.

Injectable PRP (Vampire Lift)

For Skin Rejuvenation

PRP, with the platelets, stem cells and growth factors can also be injected just under the skin and in the skin dermis itself to stimulate the sub-dermal space with new collagen elastin and ground substances (also known as the Vampire Lift) This injectable PRP will help improve the appearance of texture, fine lines and wrinkles and provides a natural way for the body to stimulate self into a more youthful appearance. Injectable PRP can be performed on its own, or in conjunction with topical application of PRP together with micro-needling, fractional laser resurfacing or fractional RF resurfacing.

Figure 8. PRP can be injected in the scalp of male and female patient suffering from Male and Female pattern alopecia and can result in the stimulation of dormant follicles and more hair density. The PRP can also be combined with Low Level Light Therapy and topical hair care products.

PRP, with the platelets, growth factors and stem cells can be injected into the scalp of Toronto male pattern hair loss and female pattern hair loss to stimulate increased hair growth and enhanced hair density. The platelet rich plasma, or PRP contains very important growth factors and stem cells that have a proven ability to stimulate re-growth of damaged and dormant hair bulbs to product hair shafts which, will in turn, increase the density of the hair. As well the thickness and quality of the hair produced will be significantly improved in the majority of cases. PRP for hair growth can be combined with Low Level Light laser therapy and Medical Hair care products to provide a non surgical, non hair transplantation option for hair growth and hair densification for men and women.

Figure 10. Female patients suffering from thinning hair can also respond very well to PRP injections in the scalp. Dormant sleeping hair follicles can be stimulated by the PRP to grow new, thicker hair, adding the densification.

Significant improvement in hair density if properly selected is usually seen by most patients with moderate male pattern or female pattern hair loss. Very advanced male and female pattern hair loss generally requires hair transplantation and PRP can be used after follicular unit NeoGraft or ARTAS robotic hair transplantation to enhance the results. Our FUE Hair Transplantation page can provide more information on male and female linear scar-less hair transplantation.

PRP Pre-Procedure Questions

Are There Any Risks Associated With PRP?

Short-term risks of PRP include occasional and mild swelling and bruising, but this is self-limiting and improves. One of the risks of PRP is that patient’s expectations exceed what the PRP can deliver. PRP is certainly not a facelift, blepharoplasty or excisional procedure. It does not replace energy-based devices such as non-ablative RF devices or fractional resurfacing, but is a very significant and noticeable adjunct to those SpaMedica energy based treatments and a way of using the patient’s own cells growth factors within their own blood to additionally amplify and improve the rejuvenative and enhancement treatment.

There is a theoretical risk of PRP and the injection of stem cells and growth factors into areas of the scalp, face, neck, skin and reproductive area may stimulate localized pre-cancers to become cancerous lesions of the skin. This is a theoretical risk and has never been documented with the use of PRP and injectable stem cell therapy, which has been available for 2 decades and certainly long enough to prove if this was a significant risk. No regulatory body in the industrialized world, not the FDA, Health Canada, CU or any other country, views patient derived platelet rich plasma, stem cells and the growth factors within this when injected into the skin and subcutaneous tissue a risk in any significant way, shape or form. Dr. Mulholland and his team are experienced at identifying and treating pre-cancerous and cancerous skin growth and these would be managed by Dr. Mulholland and the SpaMedica Dermatologic team prior to a PRP treatment

Can PRP assist in enhancing Male Sexual Function?

The use of PRP has also been demonstrated to improve the andropause related mild male pattern erectile dysfunction. This combined with a typical physician prescriptive such as Viagra and Cialis can improve sexual performance.

Can PRP be combined with Fractional Laser Resurfacing?

PRP is an ideal adjunct to Fractional laser or radiofrequency (RF) resurfacing. The Fractional laser and RF devices can themselves lead to the best possible wrinkle acne scar reduction and excellent skin tightening. The addition of PRP amplifies the collagen production from the laser and RF thermal injury leading to a better result and speeds up wound healing and recovery from the treatment.

PRP treatment optionsFigure 1. A. Fractional CO2 laser resurfacing creates a thermal ablative lesion in the skin and skin tightening. B. PRP can be applied topically through the Fractional laser defects giving the PRP direct access to the under skin element. C. The PRP combined with the Fractional Laser resurfacing and/or Fractional Radiofrequency resurfacing can lead to the best possible wrinkles reduction, acne scar improvement and skin tightening

Does PRP Work Best With Micro Needling, Fractional Laser or Radiofrequency Resurfacing?

PRP has been proven to amplify the result of any fractional injury to the skin. Fractional non thermal techniques, also called micro-needling can enhance the appearance of the skin and the addition of topical PRP with amplify this result. The results of Fractional laser and Fractional radiofrequency resurfacing on the skin adds the element of thermal ablation and thermal contraction and leads to better wrinkle reduction and tightening compared to micro needling, but also has more redness, recovery and downtime. By adding PRP to the fractional laser and fractional RF injury, studies have shown the healing time is better and the amount of new collagen and skin rejuvenation is improved.

PRP treatment optionsFigure 1. The use of Fractional lasers and Fractional Radiofrequency resurfacing devices, combined with PRP can greatly amplify the reduction of even the deepest wrinkles an acne scars.

PRP Post-Procedure Questions

Is There Any Downtime or Recovery After PRP Treatment?

The PRP procedures, Vampire Facial, Vampire Lift, PRP Hair density injection and PRP Vaginal rejuvenation injections themselves may be as quick as several minutes for peri-vaginal or penile injection, or as long as 15-20 minutes for male pattern hair loss and anti-aging aesthetic skin treatments. Generally, the anti-aging aesthetic treatments are performed after micro needling, or fractional laser or RF resurfacing the skin prior to application and then sub-dermal injection follows so anti-aging treatments are approximately 30 minutes in length. Most male and female pattern hair loss treatments will require 30-45 minutes to complete.

Following the PRP treatment, Toronto PRP, Toronto Vampire Facial, Vampire Lift and PRP Hair and PRP Vaginal rejuvenation patients will experience a minor degree of swelling, inflammation and occasionally bruising in the region. PRP Hair loss and PRP vaginal rejuvenation patients can return to normal activities immediately. Same with peri-vaginal and penile PRP injections. Topical and injectable (Vampire facial and lift) anti-aging PRP treatments after fractional micro needling require 24 hours to recover from the minor redness and bruising. For PRP topical and injectable patients performed together with fractional Laser resurfacing or Fractional radiofrequency resurfacing more downtime is required related to the fractional resurfacing portion of the procedure. The fractional laser and fractional radiofrequency resurfacing and PRP patients need 3-5 days of downtime for moderate resurfacing parameters and, for more aggressive treatments, 7-10 days of downtime. The PRP is simply an adjunct that’s applied topically to gain access to the skin through the fractional ablative lesions for a more significantly enhanced collagen production. PRP has also been proven to hasten the speed with which fractional laser and RF resurfacing patients heal an to heighten the collagen production.

Sub-dermal injections and intradermal injections of PRP are reasonably comfortable and very little downtime measured in 1-2, with the only significant risk being mild bruising.

What Kind of Results Can I Expect Following My PRP Treatments?

PRP is a very appealing process for patients, as it takes the patients own stem cells and growth factors from the their own blood and body and transfers those elements to areas where enhancement is required either in the scalp, the skin of the face, neck or body or vaginal lining and or penile tissue. The results following PRP can be quite noticeable to impressive depending on the condition being treated. Taking advantage and leveraging the bodies own ability to use it’s own stem cells and growth factors to stimulate normal human differentiation to a more youthful state is a desirable outcome and the goal of PRP.
