What is EnerJet™?

EnerJet™ is an extremely unique technology that allows a needleless delivery of Juvéderm®, Restylane®, Voluma™ and other injectable soft tissue products.

Figure 1.  EnerJet is a gentle, needle-free propulsion delivery technology where products that could only formerly be injected with a needle can now be delivered into and under the skin in a super accurate and painless propulsive spray using the: A. Enerjet device and B. Enerjet Hand piece.

The EnerJet technology uses a powerful, but gentle, needle-free propulsion technology where a kinetic propulsion of fluid transmits the delivery of the selected product across the skin barrier without the use of a needle or anaesthesia.  The EnerJet™ has a diluent or diluting solution into which Dr. Mulholland or his healthcare professionals will mix Juvéderm® Ultra Plus or Juvéderm® Plus, Voluma™, Restylane® or other soft tissue fillers. Dr. Mulholland and the other physicians may even dissolve tissues that will break down enlarged scars or rejuvenate the appearance of the skin or bleach hyperpigmentation or dyschromia or melasma.  Once the product is dissolved into the diluent syringe, the syringe is placed into the EnerJet™ handpiece.  The handpiece emits a powerful, but very gentle, kinetic spray.  Millions and millions of tiny dissolved particles of active product are then propulsed across the epidermal surface into the superficial and mid layers of the undercarpet of the skin, called the dermis.  For a surface area of 1,000 times the entry point, the micro-particles of injectable product are distributed for a 2-centimetre radius within the skin.

Figure 2.  The EnerJet emits a powerful, but painless and gentle needle-free kinetic injection that delivers the compound across the skin into the deeper layers of the skin much more accurately and delicately than could be achieved with a needle. The depth into the skin and soft tissue can be set and the jet spray creates even channels using barotrauma and fluid in an even fashion, radially from the injection site. The circular pattern of spread is 1,000 times the entry point or approximately 2cm.  The epidermis, or surface skin is left intact with a small 200 micron entry point. The body then heals the 1000’s of micro tunnels in the dermis with new elastin, collagen and ground substances leading to more youthful, smoother and tighter appearance.

The exquisite sophistication of this product results in the most superficial and accurate injection possible.  The precision and superficial spread of the injection of these products using the EnerJet™ can never be replicated with the human hand or the human eye.  The small, little micro-barotraumas and tunnels that occur through the dermis will stimulate your own collagen, elastin and ground substance reproduction.  The Juvéderm®, Restylane® and other products that are used there to space open the tunnels while the micro trauma healing occurs amd dissipate with time but are replaced fully with your own collagen tissue for a good long-term improvement.

How is the EnerJet™ treatment performed?

After a history and physical examination, appropriate candidates are selected for the EnerJet™.  If you are a candidate, the appropriate product – Juvéderm®, Restylane®, Perlane®, bleaching agents and/or bio-stimulatory agents – are mixed with the diluent in the syringe and attached to the handpiece of the EnerJet™.  No topical or injectable anaesthesia is used and the powerful, but gentle, jet is triggered, expelling a propulsive micro-spray across the epidermal surface; the entry point penetrating the active target.  The active target may be a wrinkle, skin laxity, acne scars, skin irregularity, stretch marks or hyperpigmentation and melasma.

A series of these EnerJet™ treatments are then triggered across the active treatment site until all areas are covered.  Generally, one to two syringes are required to treat most active areas, such as the cheeks, the face, neck or stretch mark regions of the body.

Who is a candidate for EnerJet™?

Candidates for EnerJet™ are those patients that are dissatisfied with the appearance of their wrinkles, especially periocular, cheek and perioral, also known as smoker’s lines, crow’s feet and granny cheek lines.

Other candidates for a good outcome to the EnerJet™ volumetric remodeling are patients with droopiness of the brow, cheek, jowl or neck, as well as patients with acne scars, with silvery-white stretch marks on the abdomen or other body areas and patients with pigmentation issues, such as melasma or hyperpigmentation. Because the EnerJet is not a laser, does not use heat, or radiofrequency energy, it is very safe in patients with darker skin, who would otherwise be prone to hyperpigmentation.

Thus, EnerJet Indications include:

– Wrinkles

– Laxity

– Stretch Marks

– Acne Scars

Who is not a candidate for the EnerJet™ treatment?

Contraindications for EnerJet™ needless injectable treatment would include Toronto EnerJet patients that are pregnant at the time of treatment, patients with active skin cancers in the region of the treatment, patients with unrealistic expectations or patients with active skin disorders, ulcerative lesions or infections in the areas of treatment.

Interestingly, EnerJet™ can be used very safely in patients of colour – skin types IV, V and VI and even those with risk of hyperpigmentation can respond well to EnerJet™ treatment where they might not to laser or radiofrequency therapy for fear that the heat will lead to some increased pigmentation.

What kind of anaesthetic is used during EnerJet™ treatment?

Toronto wrinkle, laxity, acne scar and stretch mark patients can be reassured that EnerJet™ needles injectable volumetric remodeling is performed without topical or injectable anaesthesia.  The treatment is very comfortable and most of the products that are deployed have local anaesthesia in them at the time of deployment anyway.  The treatments are much more comfortable than laser hair removal, photo rejuvenation treatments or other needle based injectable therapies, such as Botox injectable or Juvederm.

How long does the EnerJet™ volumetric remodeling treatment take?

Depending on the size of the area, an entire face, for brow, face and jowl or a neck kinetic lift may take 45 minutes.  Small areas, such as scars or wrinkles of the cheeks, the upper lip or the lower lids or stretch mark areas of the body may take half an hour or less.

What kind of recovery can I expect from my EnerJet™ treatment?

Toronto EnerJet™ patients with wrinkles, laxity, acne scars or stretch marks getting treatment with Enerjet volumetric remodeling can expect a quick recovery. The actual treatment leaves small welts at the small entry point of the jet propulsion spray.  These little welts generally dissipate over the course of an hour to two hours.  There will be some bruising, redness and discolouration in the areas that were treated with the EnerJet. Patients should either take one to two days off work or should anticipate cover-up camouflage makeup the day following treatment and for up to 5-7 days following treatment.  Depending on the patient and the patient’s skin response patterns, there may be some swelling and edema in the Enerjet treatment area, but the swelling may resolve quickly.  There may be small crusts at the entry point.  These usually flake off within two to three days.

What kind of results can I expect from my EnerJet™ treatment and volumetric remodeling?

To discuss EnerJet treatment results, it is important to divide the results into different clinical conditions that are being treated.  In general, EnerJet™ is a series of treatments.  Treatments are performed usually once every 3-6 weeks over a series of 3 to 6 treatments to obtain the optimal aesthetic outcomes:

EnerJet™ treatment of facial lines and wrinkles

The EnerJet™ may be used to treat Toronto Wrinkle Reduction patients complaining of crow’s feet (side of the eyes), peri-ocular lines (side and under the eyes), forehead lines as well as upper lip lines (smoker lines) and cheek lines (smile lines).  After a series of 3-6 treatments, patients can notice a 50% to 70% reduction in these fine lines and wrinkles.  The EnerJet™ may be combined with other techniques, technologies and procedures, such as intense pulsed-light or fractional lasers and even neuromodulators such as Botox Injectable, Dysport Injectable and Xeomin Injectable at the time of the EnerJet™ therapy.

EnerJet™ treatment of acne scars.

The EnerJet™ is very successful in the treatment of Toronto acne scar patients and the acne scar treatments can be quite successful.  Acne scars can be notoriously difficult to treat with transepidermal technologies such as fractional CO2 alone. Fractional resurfacing from the top down, is important and one of Dr. Mulhollands’s and SpaMedica areas of expertise, but reliance on Laser and or Radiofrequency resurfacing alone to get the best acne scar improvement can be limited, because the acne scar is often very deep.  Deep treatments, like the EnerJET can be very effective as they get to the base of the Acne Scar without epidermal trauma or thermal injury and heal the acne scars “from the base up”. At SpaMedica, Dr. Mulholland and his medical team will often combine the superficial and deep treatments, such as under-the-skin injectables and Intralesion injectables that can be supplemented  with the EnerJet™ treatment combined with Fractional Laser and Fractional Radiofrequency technologies to work on the Acne scars from the “top down”.  EnerJet™ can be used to treat acne scars alone, with excellent results, or in combination with many other SpaMedica Acne Scar treatments.  Generally, for rolling hill acne scar deformities, 50% to 70% reduction in the appearance of the acne scar can be achieved with a series of EnerJet™ treatments combined with other technologies, such as fractional radiofrequency resurfacing, fractional CO2, erbium laser technology and subdermal heating techniques, subcision or even soft tissue fillers and “under the skin tighteners, such as Facetite”.

EnerJet™ and the treatment of Melasma and Hyperpigmentation

EnerJet™ can be used successfully to deliver bleaching agents through the epidermal/dermal surface into melasma to minimize its ongoing discolouration.  EnerJet™ can be used alone in the treatment of melasma or discolouration or combined with intense pulsed light or low-fluence, ultra-short pulse duration lasers, such as the PicoSure® or the RevLite™.  Generally, 30% to 70% reduction in the appearance of melasma can be achieved, with this SpaMedica combination therapeutic approach. The high-tech SpaMedica devices listed above, include the EnerJet and do not use heat to treat the melasma making reactive worsening (PIH, or post inflammatory hyperpigmentation) far less likely.

EnerJet™ and the treatment of stretch marks

White, silvery, atrophic stretch marks or dark stretch marks in dark-skinned patients are amenable and can respond well to EnerJet™ treatments.  The EnerJet™ cannot completely remove the appearance of the stretch marks, but the texture and superficial appearance can certainly be improved and patients have reported 30% to 60% or more improvement in the appearance of stretch marks following a series of EnerJet™ treatments.  Of course, stretch marks can benefit from combined therapy such as fractional radiofrequency resurfacing, fractional CO2 resurfacing, other needling technologies, subdermal heating techniques such as FaceTite™ combined with a series of EnerJet™ treatments.Keloid Scar Before Enerjet

What are patients saying about their EnerJet™ treatments?

EnerJet™ needless treatments result in a long-term production of a patient’s own collagen, elastin and ground substances or resolution in pathologies such as hyperpigmentation or melasma.  Thus, the EnerJet™ is used to jet-remodel the body’s own tissues and the patient’s own tissues last substantially longer than other injectable products alone.  Results from a series on EnerJet™ needless injectable treatments may last several years; however, standard maintenance treatment with the EnerJet™ technology or other non-ablative bulk heating technologies or fractional technologies or intense pulsed-light technologies is often required to protect your investment.

Are there maintenance treatments required after finishing my EnerJet™ treatment program?

Once a series of treatments of EnerJet™ needless injectable volumetric remodeling has resulted in the optimal improvement in your clinical concerns – be they wrinkle reduction, acne scars, laxity, lifting, stretch marks, raised or flat scars, or melasma – then there is often a maintenance requirement, or a “protect your investment” single treatment once every three to four months to optimize and maintain the results you have achieved.  It may not only be the EnerJet™ needless injectable that is used to maintain your improvements, but other, simpler technologies with no down time, such as bulk heating RF technologies such as the Venus Freeze, Exilis or Forma™ or low-down time fractional technologies such as Intracel™, MicroPen™, the eMatrix or the Fractora™.   Intense pulsed light or the PicoSure® may be used to maintain the results of your melasma and other non-ablative RF tighteners and fractionals can be used to maintain the results of your stretch marks.  Occasionally, a treatment once every year with an EnerJet™ volumetric remodeling to stimulate new collagen, elastin and ground substances may be required once every 12 to 24 months as the remodeling that was achieved with the original EnerJet™ treatments subsides and breaks down with aging and time.

Can the EnerJet™ be combined with other technologies?

EnerJet™ is often combined with other technologies to synergize the overall improvement.  EnerJet™ needless injectable works through a transepidermal delivery of  a micro-barotrauma and needleless injection of product that dissipates throughout the intermediate, mid and deep dermis.  Technologies that use thermal transepidermal resurfacing, such as Fractora™, fractional laser and radiofrequency resurfacing, eMatrix, fractional CO2, Fraxel®-type infrared and fractional technologies or bulk heating radiofrequency technologies, such as Venus Freeze, intense pulsed-light Photo Facial technologies and other laser technologies, such as the PicoSure® and the RevLite™, or subdermal heating techniques such as SmartLipo™, FaceTite™ or ThermiRF™ may all synergize the treatments and results achieved with the EnerJet™.  It is best during your consultation with the SpaMedica consultation coordinator, or, Dr. Mulholland himself, to explore the combination treatments that may work well with the EnerJet™, which may be simple soft tissue fillers and neuromodulators, such as Botox Injectable, or other laser, light and energy devices.

Usually, the combination of technologies as recommended give the best possible result and EnerJet™ needless injectable may be a very important part of that combination therapy.

How much does an EnerJet™ treatment cost?

The cost for EnerJet™ needless injectable treatment depends on the size and the number of syringes required to treat the pathology.  Whether it’s a total brow, face or neck kinetic lift or localized upper lip lines, acne scarring of the cheeks, stretch marks of the abdomen or other body parts or melasma, the cost will depend on the number of syringes deployed.  In general, the cost per syringe is approximately CAD $750 per syringe.  For multiple areas, cost per volume value pricing is deployed.
